The Hospital from Home Program at Hackensack Medical Center in Hackensack, NJ

I wanted to share with all of you this wonderful program from Hackensack Medical Center in Hackensack, NJ. While you must past an eligibility for this program, I think this innovation will change the way hospital visits will work in the future for certain patients.

Welcome to Hospital From Home

The Hospital From Home program combines the best of hospital-level care with the comforts of home for patients with specific conditions that meet program criteria. Hospital From Home is a care model that brings the expertise, services, equipment and amenities you need for treatment and recovery to your door. The program may be available to some patients as an alternative to being admitted to the hospital, and has been proven safe, effective and patient satisfaction and experience with the program has been exceptional.

Eligibility:

If you are eligible for the program and would like to participate, there will be a medical evaluation and assessment. Once admitted to the program, all of the below services will be provided including:

  • Two in-person visits from a nurse every day
  • Daily virtual visits from your attending physicians
  • 24/7 access to our Care Command Center team staffed by nurses
  • Home monitoring and communications technology (delivered, set-up and training on the one-touch, user-friendly communications console)
  • Remote monitoring of vitals signs
  • All medications (including IV and infusion therapy) delivered to your home
  • Rehabilitation services (including physical, occupational and speech therapy)
  • In-home diagnostic services including lab work and imaging
  • Durable medical equipment and supplies including beds, safety and stability aids, wound dressing and more
  • Care management and planning with social workers
  • Respiratory care including delivery of oxygen
  • EMS transportation for urgent care if needed
  • Meals and nutrition
  • Courier services
