Adult Day Health Care-Housing Works Healthcare

Housing Works offers an Adult Day Health Care program for Medicaid eligible members (18+) of our community who are living with or at risk for HIV. Our team of professionals works closely with members to address substance use, mental health concerns and/or difficulties adhering to medications/treatment. Utilizing a harm reduction approach clients can set a series of small, achievable goals. Overcoming health and wellness barriers is made possible in a supportive community setting.

All clients are assigned a primary case manager and have access to bilingual and interdisciplinary staff comprised of experienced social workers, creative arts therapist, nutritionists and nurses.

Services include:

*Supportive Case Management

*Individual and group therapy

*Medication Management

*Substance Use Counseling

*Nutrition Counseling

*Hot, Nutritious Meals (breakfast and lunch)

*Holistic care (acupuncture and reflexology)

*Creative Arts Therapy

*Transgender services and support groups

*Recreation-Group Activities/Field Trips

We also offer direct in-house referrals to:

*Primary Care and Behavioral Health

*Hep C treatment program

*Health Home case management

*Peer vocational and training opportunities

Eligibility: Adults-18 and older, who are Medicaid eligible and are living with or at risk for HIV.

Hours of Operation: We are open 365 days a year, including major holidays.

Monday-Friday: 8:30am-4:00pm and Weekends: 8:30am-3:00pm
