What is Support Coordination?

*Assessing the needs, wishes and desired outcomes of each individual.

*Developing an Individualized Service Plan (ISP) along with the individual, their family and those identified as part of the team.

*Locating the supports and services identified in the ISP so as to meet the needs of the individual and their family.

*Linking the consumer and family with service providers and generic community resources.

*Coordinating the identified supports and services as requested by the individual and their family.

*Monitoring supports and services to assure that the ISP is implemented and continues to meet the needs and desires of the individual, while their health and safety are assured.

*Maintaining eligibility information and assisting individuals in applying for the benefits for which they are eligible.

*Who is Eligible for Support Coordination Services?

Support Coordination Services are available to individuals registered with DDD.

*What is a Support Coordination Agency?

A Support Coordination Agency helps you get the services you need. A Support Coordinator will assist through the process of self-direction and empowerment. The Support Coordinator will provide information about the range of services available during the planning meeting.

Step 1:  If you registered with DDD and a resident of Bergen or Hudson Countries:

hip’s Support Coordinators will:

*Assist in identifying and utilizing community-based programs, resources and services to develop a plan to meet your individual needs.

Step 2: Through developing the individualized Service Plan:

hip’s Support Coordinators will:

*Assist in identifying goals and needs in accessing community-based programs and services.

*Explore the natural supports of family, neighbors and community to assist in developing and designing a plan to meet your individual needs.

Step 3: Once your plan is approved by DDD:

hip’s Support Coordinator will:

*Continue to be there every step of the way. Ongoing contact will ensure that necessary services are in place not only for today but also for the future.

Heightened Independence and Progress (hip) is a private, non-profit agency that administer two non-residential Centers for Independent Living. hip has been assisting people with disabilities to maintain their independence in the community for the past 30 years. I have started two new blog sites for the fire department, one "EngineOneHasbrouck HeightsFireDepartmentnj" for the Hasbrouck Heights Fire Department to discuss what our Engine Company is doing and the other is "BergenCountyFireman'sHomeAssociation" for the Bergen County Fireman's Association, which fire fighters from Bergen County, NJ, go to the Fireman's Home in Boonton, NJ to bring entertainment and cheer to our fellow brother fire fighters quarterly.
