AZ & Me Prescription Savings program for people without insurance.

What is it?

*AstraZeneca medicines provided at no cost

*Mailed to the home or physician’s office

*Medicines shipped when completed applications is filed

*Enrollment is up to 12 months, with opportunity to re-enroll

Eligibility:

*Do not have prescription coverage through private insurance or government programs.

*Annual income at or below $30,000/individual or $40,000/couple

*US resident, green card holder or work visa holder

AZ &Me Prescription Savings program for people with Medicare Part D

What is it?

*AstraZeneca medicines provided at no cost

*Mailed to the home or physician’s office

*Medicines shipped when completed application is filed

*Enrollment is on a calendar year basis, with opportunity to re-enroll

Eligibility:

*Enrolled in Medicare Part D

*Annual income at or below $30,000/individual or $40,000/couple

*Must have spent at least 3% of annual household income on out-of-pocket costs on prescription medicines within the calendar year.

AZ & Me Prescription Saving program for healthcare facilities

What is it?

*Outpatient-based program for free clinics, community health centers and disproportionate share hospitals to obtain AstraZeneca medicines at no cost for patients who qualify

*Bulk replacement program based on facility’s qualifying product utilization

*Facility enrollment is for 2 years, with opportunity to re-enroll

Eligibility:

*Facilities are non-profit and have an on-site, non-retail, outpatient licensed pharmacy or dispensary.

*Eligible patients do not have prescription coverage through private insurance or government programs

*Eligible patients have annual income at or below $30,00/individual or $40,000/couple

AZ&Me Prescription Savings Programs

Our programs are designed to help qualifying people without insurance, those in Medicare Part D and those who receive their medications through participating healthcare facilities.

We’re committed to helping people get assistance affording their medicines, whether they’re our medicines or those made by someone else. It’s why we also participate in multi-manufacturer programs like The Partnership for Prescription Assistance.

As AstraZeneca, we believe it’s not enough for us to simply make medicines. We have to make sure that the people who need those medicines can get them. It’s why we’ve offered patient assistance programs for over 30 years.
